Medical and Prescription Plan A Preferred Provider Plan (PPO) is available to our employees through UnitedHealthcare. Plan Highlights include $20 co-pay for in-network office visits and a three-tier pharmacy benefit, including mail-order prescriptions for increased cost savings. Paid for by FNF Vision Plan A Preferred Provider Plan (PPO) is available to our employees through Avesis. Plan Highlights include $10/20 co-pay for in-network eye exams and materials. The Plan covers contact lens as well. Paid for by FNF Section 125 Cafeteria Plan Both Flexible Spending Accounts and Dependent Daycare Accounts are available providing our employees with ways to save using pre-tax dollars to pay for un-reimbursed medical and dental costs as well as daycare expenses. Life Insurance Benefits Employees are provided with a basic level of coverage for both life insurance and accidental death and dismemberment. Paid for by FNF Long-term disability is provided through Unum Provident and features a benefit of 60% of an employee's average monthly pre-disability income. Employees may qualify for company-paid supplemental coverage based on attaining certain salary levels. Paid for by FNF 401(k) Plan Our plan is currently administered through SEI Investments. With several funds available, as well as on-line tools for retirement planning, our employees are encouraged to begin contributing up to 15% after completing 90 days of employment. The Company match begins after 1 year of employment and 1,000 hours of service. A discretionary match based on Company profits may occur after fiscal year end. Health & Wellness Initiative Employees can qualify for a bonus each time they complete a component of FNF's Health & Wellness Initiative Program . For 2008, there are 3 components comprised of health questionnaires, lifestyle assessments, and completion of an annual physical with a physician. Upon completion, each of these components is worth either $100 or $200, depending on the employee's level of current medical coverage. PTO Program ( Paid Time Off) New employees hired in the current year will be eligible for PTO in January of the following year. PTO will be prorated from their date of hire through the end of the year. PTO can be used either as sick or vacation time. FNF believes a Paid Time Off Program will allow employees greater flexibility to tailor their time off to their individual needs or to those of their families *Employees with more than 5 years of service qualify for increased amounts of PTO.
